POLICIES ACKNOWLEDGMENT OF PROVIDER STATUS I understand that Ann Pollock - Art by Ann is currently a cosmetology student / soon-to-be licensed cosmetologist and is in the process of completing required state licensure hours. Certified in > APRES GEL X® • BARBICIDE® Certification — the main infection control/sanitation certification. • Be Healthy Initiative by BARBICIDE® — healthy skin/healthy lifestyle certification • Skinny on Skin —skin cancer awareness certification/course • Lucascide Disinfection Course/ Certification — sanitation/disinfection education course. I acknowledge that services may be performed as part of skill-building, portfolio development, and practice. I acknowledge I am paying for product/and or donations I voluntarily consent to receiving services with full knowledge of this status. ⸻ ACKNOWLEDGE AND INFORM I understand that beauty services, may involve potential risks including: • Allergic reactions • Skin irritation • Sensitivity or discomfort • Dissatisfaction with aesthetic outcome (Though these things rarely happen, Waivers are in place to keep service providers safe) I agree to inform Ann of any allergies, medical conditions, skin sensitivities, or prior reactions before service begins. ⸻ AFTERCARE RESPONSIBILITY I understand that proper aftercare is my responsibility and that failure to follow aftercare instructions may affect the longevity and results of the service. CANCELLATION POLICY
By booking an appointment- you agree to these terms. Late fee- 10min late 15$ fee, 10+ or more minutes late 20$ fee Anything more than 15 late results in a no show cancellation policy and fee will be processed. IF SOAK OFF IS NOT BOOKED 10-20$ FEE (depending on service) Cancellation Policy • 72+ : No fee • Within 72 hours of appointment: 50% (nail art add ons included) • Within 48 hours: 100% of service total (nail art add ons included) • No-shows: 100% of service total + 20$ fee All fees are charged to the card on file. Make sure there’s a sufficient balance. •All cancellation fees are due within 24 hours of cancellation. If payment is not completed, another 48 hour grace period will be given. After 48 hours, a $20 late fee will be added to the remaining balance. Designs default too- Simple design adds on 25$ Medium design adds on 50$, Extra design adds on 75$, 50% within 72 hrs or 100% within 24 hrs will be charged for base+design. I completely understand that unfortunately life happens and emergencies come up. However, this policy is in place to protect reserved appointment times, make sure my time is respected, ensure I'm properly compensated, etc. Because of this, I'm unable to make exceptions. The policy must be applied evenly to every client so things stay fair, respectful, and consistent across the board.
